1. Preamble
Your utilization of the MultiMedRx™ (MMRx) Telemedicine Program (“Program”),
administered by MultiMedRx™ LLC, denotes your agreement to the pursuant Terms and
Conditions.
2. Prior Consent
Subsequent to employing the Program, formal consent must be given by signing an
independent detailed consent form, acknowledging the breadth and restrictions of
telemedical services.
3. Payment Terms
By providing a credit card or other payment method accepted by us, you are expressly
agreeing that we are authorized to charge you for the services you have selected and any
applicable taxes or fees. The Patient Care Fee is $145 paid monthly. If medication needs
to be compounded and administered directly from the physician to the patient, the prices
for Semaglutide is $150 for dosage up to 1.25mg and Tirzepatide is $235.71 for dosage up
to 7.5mg. If your payment method fails or your account is past due, we may suspend or
terminate your access to our services.
4. Compliance with HIPAA
The entirety of telemedical consultations adhere to the stipulations of the Health
Insurance Portability and Accountability Act (HIPAA).
5. Services and Remunerative Transactions
Details of the services offered by the Program, as well as payment procedures, are
meticulously exemplified in the Program’s user manual.
6. User Liabilities
Your obligations include the provision of truthful medical data and ensuring secure
access to the Program.
7. Subscription Modalities
MultiMedRx™ may propose subscription schemes with varied attributes. The details for
each subscription modality are conveyed in the user manual.
8. Corporate Restrictions
The employment of the Program for commercial purposes is under the jurisdiction of
corporate practice of medicine laws.
9. Discontinuation
MultiMedRx™ possesses the authority to revoke user access, predicated on violations of
these Terms and Conditions.
10. Amendments to Terms
The stipulated Terms and Conditions, elaborated by MultiMedRx™, are subject to periodic
modifications, and the onus is upon you to review any such updates.
11. Prevailing Legislation
These terms operate under the jurisdiction of the laws corresponding to the location
where MultiMedRx™ Providers are licensed and established.
